Abstract

I. Introduction
The prevalence of Mandibular second premolar with three canals is very low and only a few cases have been reported. According to Zillich and Dowson, The prevalence of mandibular second premolar with three canals is only 0.4 % in U.S.A. population. Vertucci¡¯s type V pulp configurations have dividing canals in middle or apical or root, and are known for its treatment difficulty due to limited canal access and limited illumination. The endodontic microscope allows us to perform better root canal treatment in difficult teeth with good illumination and magnification. This case report will present a successful root canal treatment of Vertucci¡¯s type V three - canal mandibular second premolar with aid of Endodontic microscope.

II. Case Presentation
1. Sex / age : F / 22
2. Chief Complaint (C.C) : ¡° My Right lower tooth is decayed. The Local Dentist told me to visit a dental hospital. I had pain, but not now.¡±
3. Past Dental History (PDH) : Decayed state of distal wall of the tooth . Undergoing RCT.
4. Present Illness (P.I) : P/R(-), Mo(-), EPT(-).
5. Impression : pulp necrosis due to dental caries.
6. Tx plan : Root canal treatment on #45 with endodontic microscope aid.

III. Conclusion
Endodontic Microscope is a useful tool in difficult root canal treatment. The patient in this case did not have any clinical symptom or radiological sign after 6 months of root canal treatment. Difficult root canal treatment cases such as Vertucci¡¯s type V three - canal mandibular second premolar can be successfully treated with endodontic microscope aid.
